Plan Your Care app for iPhone and iPad

Plan Your Care app provides mobile app access to users of the Plan Your Care service provided by Latrobe Community Health Service.

Plan Your Care is a collaboration led by Latrobe Community Health Service in conjunction with Statewide Autistic Services and Melba Support Services. The development was funded by the Department of Social Services (previously FaHCSIA) through the Practical Design Fund (PDF). The primary purpose of the PDF is to support initiatives to prepare people with disability, their families and carers, the disability sector and workforce for the transition to the National Disability Insurance Scheme (NDIS).
